The rules state that air combat comes after flak. However, in reality the target hex interceptors attacked a formation first. The interceptors left as the formation approached the target hex so as not to be hit by friendly fire. The B17 crews knew that once the intrceptors left, flak was next.

Also, is interception automatic in the target hex, or do we go to the Formation Events Table and roll?

Rick,

The game is not so granular as to matter whether flak gets the first shot at a formation or enemy interceptors get the first crack. The point is that they can both get their kicks in (if interceptors are rolled for).

I have wondered the same thing about your other point and reached a work around through a house rule that allows for a squadron event die roll after the flak egress attack.
