What triggers a 2d6 roll for a Campaign Random Event?

How do these event checks come up in the Heroes of the Motherland campaign?

Page 43 - one Random Event per Scenario and it is activated the first time during play that both players roll the same number on their die in their combat resolution (i.e. the opposed die rolls).
